using specific email addresses with email templates

I am trying to design an email template form to automate the collection of
some data we use.
I want the email form to be sent to a specific email address and therefore
want this email address loaded into the 'To' box every time the form is used.
How can I do this?

Run the form, put in the desired To address. Publish the form.
