VBA file/save

I have a .dot that I'd like nun a macro to rename <file name>.dot to <file
name_><textbox41>.doc & set the directory to H:\my Documents and have it
appear in the und user's file/save dialog box.
I'm a novice, so please be verbose.

What sort of document is this? One that is protected for forms and
Textbox41 is a FormField? Or is Textbox41 a Textbox control on a userform?

The answer will be different for each case, but for starters, I would
suggest that you give meaningfull names to such items what ever type they
are.

Doug...
I was a developer years ago b4 VBA or MSoft forms. Textbox41 is a control,
and there is only one on the form, so I'm not concerned about naming
conventions.
Saved this as a template as an attempt to protect it.
I need to protect the template, on file/save click append the last name
field to the file name. Overwrites are ok, since two approvals are needed on
the form.
FYI...the dot will be posted on a portal for public access, so a directory
change to H:\My Documents might be a good work-around(?)
How much & turn-around?
