Viewing a form only after Date Range has been entered

I have a form with a button that opens a date range form. As of now,
you enter the date range and a pop up form opens that reveals the
data. The data in the pop up form is just 3 text boxes that have
dcount control sources off a query.

What I want to do is get rid of the pop up form and have the text
boxes appear on the same form as the date range button...but only
appear after a date range has been entered. Then I want it to
disappear again once the database is closed.

Any suggestions?
