Visio COM Addin problem

I have made a Com Addin and it loads fine whenVisio starts with my COM
addin menu appearing on the top. But when I select any option from the
menu it gives the error: System.IO.FileNotFoundException caught. The
specified module could not be found. (Exception from
HRESULT:0x8007007E)

this occurs even though I do not read from any file until later on,
after the user enters info in a dialog box. But this error occurs even
before the dialog appears when I havent even accessed a file. I've been
stuck on this for some time now trying new installs and what not so any
help will be appreciated.
