vsto 2003 suddenly cannot debug outlook add-in

i created an addin for outlook 2003. I am not using a shim for right now. I
had no problem running and debugging from vsto. But now something has
changed. When i try to run the addin from vsto, outlook loads then exits.
The next time I run outlook, i get a dialog saying that "outlook experienced
a serious error last time 'myaddin' was run". If i keep the addin enabled
outlook will load and 'myaddin' runs.

So to sum it up, I can run the addin from outlook, but cannot run the addin
from vsto to trace thru code.

Does anyone know what might be causing the problem?
