want to create a bullet with fill in the blank spaces....

I am trying to make a custom bullet so i can fill in the blanks...see bullet
below.

_____/______ /______ GDS 1.



I want to create a bullet that looks like the one above and have the option
to fill in the 3 spaces.

is this possible and how would i go about doing this?
