Want to Merge WEB CONTENT from merged hyperlink-as-data-field

I have an SQL table from which I pull two fields per record: a site number,
and the hyperlink to a chemical-data page specific to that site number. I
want to merge into word the site number and the WEB PAGE pointed-to by the
hyperlink stored as the second field of every SQL record. How might I do this
without writing my own application to look-up, handle and convert into a
display the records of my SQL table?
