I've had my website up for about 8 months now. This forum helped me a lot
when I was publishing it for the first time. I hope you can help now.
My dilemma:
The following form isn't working (always has in the past - now I get a
message that says "HTTP 404 Not Found"):
http://www.somoscapazes.org/index_files/formvoluntarios.html
While the following two forms are working:
http://www.somoscapazes.org/index_files/formescuelas.html
http://www.somoscapazes.org/index_files/donar.html
The one that isn't working happens to be the one that usually gets the most
use and is at this moment the most important to me.
I designed the site with Publisher 2003. I have always used FTP to publish.
Normally, to make the forms work, once I update and re-publish my website, I
simply uninstall, then re-install Microsoft Front Page Extensions, and no
problem. I have attempted this a number of times with the problem form and it
just isn't helping.
I have attempted to access the following pages, which have been so highly
recommended by David Bartosik to previous posters on similar subjects, but a
page comes up telling me "You are not authorized to view this page":

I have also looked over the following suggested pages, but I don't think
they apply to me as I've always been able to make the forms work by using
FTP, and 2 of the 3 forms do work:

Please help! There are a number of people trying to use this form at the
moment and it's important for my non-profit organization to present a fully
