What does this mean and why is it doing this?

I am trying to merge some databases and using the query
and keep getting this error.

SIS database set 0 field(s) to Null due to a type
conversion failure, and it didn't add 128 records to the
table due to key violations, 0 record(s) due to lock
violations, and 0 record(s) due to validation rule
violations.

What does this mean?

It means that the data that you are appending contains 128 records with
primary key values that are the same as records already in the destination
table.
