what form do I use to write a letter to a judge?

How do I set up a letter to a judge? I know there's a certain way it needs to
be set up in order to look professional..Can anyone help, please??

It should be an ordinary business letter. The inside address uses "The
Honorable John Q. Doe," and the salutation says, "Dear Judge Doe."
