what template can be used for creating 4in. by 2 1/2in. wallet car

I need to create laminated 2 1/2in. X 4in. laminated player cards for parents.
to include:
Player Name & No.
Parents names, cell No's, home No's

Uncertain if I need to create one from scratch, if so how (or directed to
how) and needed to be printed on both sides of cards.

What program are you intending to use? There are labels about the size you are
asking about.
