Wheere do I find a Word7 expert to hire to make me a form?

Want to hire someone to take my form and add a needed procedure. Presently, I
enter a diagnosis and then have to go to another field and enter the
associated code for that diagnosis. I would like for the code to
automatically appear in its field when the diagnosis is entered. and populate
the few other places in the form it has to appear.I can do it for things like
the name, ID number, etc but can't get this to work. I'll gladly pay the
going rates!! I know nothing about code and too old to learn so would like it
to be in a recipe-like answer (do this, and that, so I can follow the
instructions!!)

I need some extensive expert help on a project. Overall, the project
(cheese). is quite small and simple but has some complications that I
can't handle yet.
In a netshell:
Act I
An Excel WB with 3 tables in sort of a relational database
Table 1: Column 1: All 50 States: Field 8: One of four Regional
Centers for each state: Field 9: One of nine Regional Centers for
another program
Table 2: Column 1: The four Regional Center names: Field 2: Complete
multiline address for the Regional Center
Table 3: Column 1: The nine Regional Center names: Field 2: Complete
multiline address for the Regional Center

Act II
Enter, stage left, a Word Template
I have already worked out having a form open when the template is
used to start a new document. The form has a list populated by Table
1, Column 1, and another list that displays the correct Field 8 based
on which State in Listbox1 is selected. So far, so good. Now I need to
also pull in the address based on the Regional Center and then have
the address fill a form field or bookmark or something so it will
appear and print on the document. Other, than that, all I need is a
macro to validate a Word Forms Textbox upon exit. There are actually
two templates but they are so similar that I am sure that if I can get
the solution to one I can make the appropriate substitutions to do the
other one myself.
