When entering a form I get asked for a username and password

I have a Windows 2000 server with FP2002 extensions. I have a simple form
asking the person to enter name, phone, email etc. When clicking Submit they
are asked or a username and password. I'm sure this is a permissions isue on
the _private folder but am not sure what permissions I need to set. I'ved
giving Write and Modify permissions to both the SERVER\IUSR_SERVER and
SERVER\ANONYMOUS LOGON user but still have not luck. What permissions do I
need to set? Do I need to restart any services after making the changes
(wouldn't have thought so)?

Any advice would be appreciated.

Make _private folder must have read/write/change permissions. Try running the Server Health Check
and let FP fix the permissions.

I ran the Health Check and it says it can tighten security but every time I
rerun the check it always says this. I've run the "Reapply file security
settings" but still can't get it to let an anonymous user Submit the form.

Which user needs the read/write/change permission? The SERVER\IUSR_SERVER
user or someone else? If I apply it manually to the user that needs it then
that should get it working.

The IUSER_Machinename account need the permissions.

You can also work around this issue by not storing any result from the form in the _private folder,
and just have it sent to an email address.
