When I generate an index, I get an error

The index generates OK, but under X, I get Error! Not a valid document
self-reference on page 1. I checked all of my page ones - this is a multi
file document. And I can't locate the error. I even tried deleting all the
page one text and regenerate the index and still I get the error. Any
suggestions?
