You may need to check with your host. When your form submits, it gets
processed by the FP Server Extension. The server extensions have verious dll
(or exe's depending on the platform) files that perfom different actions
such as acting as a form handler. It could be that the permissions are
actually incorrect on the server extensions. This would cause them to
request a username and password from the web server in order to process the
form. Ask them to check the permissions after making sure that you
definitely aren't saving the results anywhere such as a database or file.

That might happen if the thank you page were stored in a folder that starts
with an "_", for example "_private"

That would only happen if the folder is _private.
