Where are the default directories for fax cover sheet?

I downloaded a new fax cover sheet from Microsoft. Dropped in all of my own
artwork and saved as template. When I try to fax and use it, it is not on the
lists of available cover sheets.

Why?

Wrong directory, huh? Well then where is the default directory for fax cover
sheets?

Your template needs to be placed in the Templates folder for it to
appear under File/New. To find the location of your Templates folder
go to Tools/Options/File Locations.

Sorry, guess I wasn't real clear about the problem.

When I attempt to fax from Office I click print, I drop the menu and choose
the fax option. The "Welcome to the Send Fax Wizard" pops up.

I fill in the To: and the number. When I click "Next" I get an option to
choose a cover sheet. The options are: urgent, confident, fyi and generic.

I would like to make my own cover sheet one of those options. What directory
or extension is on those cover sheets--where are they stored by default?

It's been so long since I've used Microsoft Fax I'm not sure. I'll
need to install it and check it out but I need to know which version
of Office you are using.
