where can i find a format for paper to be notarized?

i am sending my son with his grandparents and was asked to write a paper say
that i was allowing him to travel with them and then to have it notarized
but i dont know how to?

There is no special format for this. Just write it as if you were writing a
letter. Leave some space at the bottom, at least 3 inches, for the notary to
place a stamp or seal.
