Where can I find a summons template with number lines,

I am looking for a template for typing out a summons document that noties a
defendant that a lawsuit has been filed and an as apprearance must be made
before the court, at a special tine, to answer questions, and alos have it
numbered, each line numbered. I cannot find one, can anyone please help,
