Where can I find the template for "Jeopardy"?

I am a middle school teacher and I am looking for the template for the
"Jeopardy" game to use as a review tool in the classroom. I took a graduate
class and another student used it in their presentation, but I have been
unable to find it. Please be specific as to wear I can locate this template.

There are about a ton of Jeopardy-style templates for PPT. (I assume you're
talking about PPT, anyway.)
