Why does saving an htm file sometimes create a header file?

On a recent save of an htm document in Word 2003, the save created a
sub-folder containing a header.htm as well as a filelist.xml. Other similar
documents do not do this. The problem is that we're using wincvs for version
control. This generates an error when I commit the htm file in wincvs. When
my teammates try to open the file that I committed, they get an error about
the missing header file. They can go past it, but it's irritating--and I
don't know if there will be other problems later on, for example if I'm not
here and they have to make changes and save a new version. (There is no page
header or footer on the document, so it doesn't relate to that as far as I
can tell.)
