Why is my contact form not showing the Activities Tab?

When I open a contact form, I have a General Tab and an Activities Tab but
the Activities Tab is missing? Can you help? How can I show the Activities
Tab?

If this is a custom form, did you turn off that page? If not, please provide a more complete description.

I am not aware of this being a form. How can I tell? And if it is a form,
how can I turn it off?

Every item appears in a form when you open it. A published custom form will be identified with a Help | About This Form dialog.

We're still waiting for you to give a more complete description of the problem, which so far doesn't fit any known issues.

This is not a custom form. It is simply a contact form that only has a
general tab and details tab and it does not have an Activities Tab. I am
trying to identify why this is and identify a solution.

And now you've provided us with some the answers we were looking for that would allow us to have the full picture of the situation (except for your Outlook version, which you never mentioned).

Unfortunately, I have no solution to offer. I have never seen or heard of such behavior from the standard contact form. As a long shot, you can try clearing the forms cache -- Tools | Options | Other | Advanced Options | Custom Forms | Manage Forms | Clear Cache -- but I doubt that it will help.
