Why won't the form load for some people

I've published several forms to a public folder
(essentially copies of each other with a few details
changed) but, for some reason, when some users load one of
the forms (Action|New FormName), it doesn't install but
pulls up a blank standard form for the folder. I've
cleared the cache from within Outlook, deleted the cache
manually from the user's local profile, copied the form to
their personal folder, all with the same result. For
other people, the form loads without any problems. For
the same person, other forms within the folder install
fine.

Outlook 2002/Exchage 2000
