Wierd Delivery to the following recipients failed Incident

Outlook2003, Windows XP Sp2, Exchange 2003 Server.

Hello:

A user sends an email out using Distribution list EVERYONE at XXXX.

She edit some people off the list by clicking the + sign infront of it and
send it out.

Recievers got all the msg but our CEO decided to "REPLY ALL" and get error
message and NO Email was going out. attachments come w/ the rejection notice
as well...

/O=XXXX/OU=FIRST ADMINISTRATIVE GROUP/CN=RECIPIENTS/CN=JOE

and the attachments has

Final-Recipient: x-ex;/O=XXX/OU=FIRST ADMINISTRATIVE
GROUP/CN=RECIPIENTS/CN=JOE
Action: failed
Status: 5.1.0
X-Display-Name: Joe Joe

Can anyone tell me what's wrong and any way to prevent this?
