Wizard-like Fill-in forms?

I want to build a legal template for lawyers. I'd like to have a bunch of
fields at the top of the document where the lawyer can type in information
like AFFIANT, DEFENDENT, etc. Then the values entered into these fields
would be automatically substituted into the rest of the document body.
Ideally, I would be able to automatically repeated sections. For example,
say we were suing for multiple bills, we could say "4 bills" and have 4 sets
of prompts generated to gather data for each of the 4 bills in dispute.

Any help would be appreciated. I feel that this must be something very
simple to build given how sophisticated Word is, but I can't make heads or
tails of the build-in help.

I have the same need. I wish to build one for “Affidavit of Process Serverâ€
and attached said form to my Microsoft Small Business Website so clients can
fill out and then print or email to me.
