Word 07 - Creating Fax Template

I'm new to Word 07 and I need to create a Fax Cover Page Template that
I can call up from time to time, fill out and prepare to send a Fax.
How do I accomplish this in Word 07?

You can start with one that's close to what you like and modify it.
