Word 2000 Mail Merge Problem

I want to send out multiple mailings using mail merge.
I've made a data base with names, addresses, etc. I've
selected envelope style. I selected the fields I want to
print on the envelope (Name, title, Company, etc.) Then
I merge. When I view the envelopes, they do not appear
the way I want them to. The address is centered;
however, it's missing zip code (I have it included in the
database and this field is selected)--and it is repeating
city and other info on both the left and right side of
the envelope. There is also a comma before every city. I
have no commas in my database. Please help me.
Something is wrong. thank you. susan

I was going to ask if this were Word 2002 or 2003, then saw
in the message subject Word 2000. Very odd, indeed.

What's your data source, more exactly? You say "data base",
but in which application did you create it?
