Word 2003 -- Pleadings -- Header Printing Problems

We use custom designed pleading templates that place number lines on the very
left of the page in a table that is a header so it will repeat every page.
For some unknown reason, the last two lines (numbers 27 and 28) of the header
and table borders will not print, even though they appear in print preview
mode. This is limited to word 2003. The same documents print just fine in
Word XP.

Any one have any ideas?

The paper size in the printer is not the paper size Word thinks is in the
printer.

For example: if the document is set up for A4 and Word thinks the printer
contains A4, but someone has put Letter paper in the printer, the last two
lines won't print.
