Word 2003 TOC list chapter name, number, and page number

I want to create a TOC that looks like this, but I can't get it to insert the
chapter (Article) numbers ("Article" and "Page" are manual column titles, not
part of the TOC field):

Article Page
Term I 2
Cancellation II 3
Territory III 4

To be included, the article numbers would have to be part of the page number
(inserted using "Include chapter number" in the Format Page Number dialog).
