Word 2004 printing always a "report" after each documents

Operating System: Mac OS X 10.5 (Leopard)
Processor: intel

Every time I want to print a Word document, I also print automatically a "report" like that on EVERY word document... It's very annoying !!

NO PRB with any other programs.. excel or entourage... ONLY with word2004

EX:

Filename: Julie Heffernan shipping bids for delivery.doc

See Word | Preferences | Print, and uncheck "document properties" (under
Include with Document)
