Word 2007 User Forms

I am creating a template in Word 2007 for letterhead that has a text box for
the name/email address/phone number etc. for the employee creating the letter
in the left margin. I would like to use a fill-in field or user form so that
when a document is created from the template, a drop down list opens for the
user to select his/her name from the list. Once the name is selected, the
other information (email address, phone number, etc.) for that employee will
automatically populate the text box in the left margin appropriately.

What is the best method for approaching this task? Thank you,
