The answers to your questions are: "A side-heading is a pain in the "A"
invented by FrameMaker to make Word look bad"; and "with great difficulty"

A side-heading is a heading that is contained entirely within a space on the
left of the body text. The body text needs to be set to the right to make
room for them.

In Word, you first create a set of body styles that sets all your text to
the right. I would start with 4 cm so you have room.

Then, you customise your Heading 1 through 9 (or however many you want to
use) to set their text in a "Frame". A "Frame" is similar to a text box,
but it can be defined as part of a style, and it is in the text layer so the
TOC and Index generators can see it.

You set the frame to hang to the left of the Body Text, and ensure that you
remove the default box border from it.

When you want one, format a paragraph as, for example, "Heading 2". The
Frame will appear, and you can type in it. The frame needs to be positioned
to the left of, and level with the top of, the body text paragraph.

Side-heads have gone out of fashion in modern publishing (largely because
they're a pain and a fiddle to do, unless you are using FrameMaker).

Most people using Word use "Out-dented" headings. These are Word's
"natural" way of doing things. Readability research shows they perform
better than side-heads in enabling readers to scan and remember text. And
they are far, far easier to produce and maintain.

To produce out-dented headings, simply off-set your body text.
