Yes, you can do this using bookmarks and cross-references.
Make sure your system is set up to view hidden text and
paragraph marks by....
Tools | Options | View: All formatting marks...
Next, while your document is unprotected, select the the
input field with your mouse (click and drag over the
dots). Click, Insert | Bookmark. After the dialog pops up,
give your new field a name and click Add.
Go to your header and position your cursor where you want
the information to appear. Click, Insert | Cross
Reference: and in the Reference Type drop-down, select
Bookmark. Make sure the Insert reference to reads Bookmark
Text. Locate your bookmark name from For which bookmark.
Highlight it, and click Insert.
That's it. The information will appear after you update
fields.
