A bug in version 11.5604.5606, that didn't exist in Word 10, hides the scroll
bar in the cross-reference dialog after clicking out of the dialog (whether
an insertion was created or not). In versions 10.x-, I could continue to mark
(or highlight) a new entry point, return to the dialog, scroll to the new
X-ref and click Insert. I could continue repeating this procedure
indefinately.
Now, I must close the dialog following each click out of it because the
scroll bar doesn't appear. When I reopen, it appears for one more current
attempt.
