word template using addresslayout, bookmarks and user form

I'm looking for a base for a template (for example a letter) to incorporate
using the addresslayout (including company name), which I can customize using
my own bookmarks and field switches, i.e. Site Field would be the same as the
company name and the address. Description of work: To be able to type this in
in a form.
I would like to be able to choose different templates, i.e. choice of
letter, estimate etc.

This would be similar to the letter wizard in Word, but would show the
addressbook from Outlook and be able to choose it, choice of template - say a
radio button and a few text boxes for the bookmarks (which you could
customize - depending on the template you choose).

I have seen this type of User Form in WordPerfect before (bespoke), but have
never been able to track it down in as a Word Macro/User Form.

Does anything exist out there?
