This information is stored in the Registry under
HKEY_CURRENT_USER\Software\Microsoft\Office\10.0\Word\Data\Settings

If you have permissions to update the Registry key it could be
corrupt. Exit Word and rename Settings to OldSettings. Word will
create a new one upon starting.

Note this key holds the majority of your user preferences found in
Tools/Options and Tools/AutoCorrect Options so if this corrects the
issue and you have made modifications you will need to make them
again.

As always, prior to making any changes to the Registry you should
review "Backing up the Registry" and "Restoring the Registry" in the
Registry Help.

In this situation you may want to use the Registry/Export Registry
file command, export your selected branch, and delete the Data key
instead of renaming the Settings value. That way if you find the
change wasn't necessary or incorrect, you can merge the deleted branch
back to the Registry by double-clicking the resulting file.

Tried this - did not help... After restarting Word couple of times
User Info still disappears from the Tools|Options| User Information
tab.

Thanks for suggestion.

Any other ideas?

BTW, also noticed that the value for Toolbars in Data key says "Unable
to read information" - does that relate somehow to my problem?

Take a look under Tools/Macro/Macros. Do you have any macros named
AutoExec, AutoOpen, Document_New, or Document_Open?

I'd say the AutoExec macro is modifying your personal preferences when
Word starts. The changes are saved to the Registry when you exit Word
but the AutoExec macro is changing them when you start Word.

In the Macros dialog box change the "Macros in" drop-down to the
various available templates to determine where the AutoExec macro is
stored. (The others do not automatically run - I suspect they are
called from the AutoExec macro)

Chances are you will find it in a global template which will be found
in a Startup folder. The location of the Word Startup folder can be
determined by going to Tools/Options/File Locations. The Office
Startup folder is located in the Office installation path.

Once you find the template move it out of the folder to prevent it
from loading when Word starts.

Of course if you are utilizing the additional functionality in the
template then chances are you'll have to live with the changes it
makes each time you start Word. If you are able to modify their macros
then it could be rectified but most third-party manufacturers protect
their code.
