wording to reprimand for unauthorized compnay vehicle use

I don't think a template with wording this specific is available. You might
try searching Google to see if you can find any samples. Otherwise something
like the following should be sufficient:

Dear <name>:

It has been brought to our attention that on <date> you used <vehicle
information> for your personal use. As outlined in our employee handbook
<insert section/page/etc> it states <cite handbook rule> and you are
therefore in violation of our company policy.

This letter is to inform you that the following disciplinary action will be
taken:

<insert disciplinary action>

If you are not in agreement with the disciplinary action cited above you may
provide us with a written statement requesting a formal hearing on this
matter. Your statement must be sent by certified mail within fifteen (15)
days from the date of this letter.

Sincerely,
<name, title, etc>
