You do not have permission to send to this recipient

I have made a simple form with two response forms. The forms are deployed to
the user's personal form library. When they attempt to use one of the reply
forms they get the following error:
You do not have permission to send to this recipient

Any ideas what would cause this error?

Typically when I received this error message, it was because I did not have
permission to send as that mailbox. I don't know how this will help you
since the forms are in the user's personal mailbox, but it's a place to start.
