"You must first select icons for your form" MAPI error

I have created a personal form, fairly simple, and published it to a personal Mailbox created for the occasion. I've worked on it for a while but, last time I tried to publish it, I run into the following error message

"Unable to determine where to save this form due to an error

Then, I am prompted where to save the form. I selected my usual form name and location and got the following error message

"Unable to successfully publish the form due to a MPAI error. You must first select icons for your form. To select icons, click the Change Large Icons and Change Small Icons buttons.

I've tried to play with the toolbar buttons or save my form under a different name, and also cleared the cache, nothing seems to get me out of here, any idea?

Did you try doing what the warning message suggested and setting the large
and small icons on the (Properties) page of the form?
