I think they probably _did_ track it down and fix it - just not in time for
the last release of Office 2001 before adopting the "pro-Apple" policy of
supporting the move to OS X by ceasing development work for Classic
products. Those bugs don't occur in Office X or 2004. Ergo, they either
fixed them when converting the code base to Carbon for Office v. X, or the
bugs just luckily disappeared by themselves when the system changed under
it. I know which version I think is more likely...

I had a conversation with someone (Rick Schaut maybe?) at the Summit who was
not aware of any fix. I believe it was either the switch to OS X which made
the difference, or some code change in Office X for an altogether unrelated
function which effected a fix by chance. I also seem to remember being told
that they could never replicate the problem; I know I can't, although I
swear it happens most often when I've been using Tools>Customize. Rick (?)
said that it's probably something that the user does which results in the
menus going missing hours or even days later. IOW nothing close to a simple
cause and effect scenario.
